If this is your first visit to the clinic it is important that we have some clinical background in order to make your initial visit useful. Documents we may ask you to provide include:
Films - x-rays, CT scans, please call your hospital's Radiology Department and arrange to pick up these films/CDs to bring with you to your visit.
Medical Records - Recent office notes from your referring pulmonologist or healthcare provider, including pulmonary function tests, and other relevant information.
Referral - A referral from your Primary Care Provider for this consultation. If your insurance does not require referrals for office visits, please notify your insurance company prior to the appointment.
