As part of our ongoing service to referring physicians, we provide the following referral options.
Our trained coordinators can facilitate consultations and second opinions. If you are unsure of which Brigham and Women's physician to contact, the coordinator can direct you to the appropriate faculty member. The Physician Referral Service is staffed Monday through Friday, 8am-5pm. After hours, you may leave a message which will be returned promptly on the next business day.
Please use this secure form to request a new patient appointment with one of our physicians. Once the form is submitted, a trained coordinator will contact the patient to schedule an appointment. Referring physicians will be provided with the appointment details.
The Online Physician Directory provides profiles of all physicians and specialists at Brigham and Women's Hospital.
With the newly-updated Partners Physician Gateway, you can monitor the status of your patients who are receiving care at Brigham and Women’s Hospital, Brigham and Women’s Faulkner Hospital and all Partners HealthCare (PHS) institutions, Dana-Farber Cancer Institute, and Massachusetts Eye and Ear, using new features that are part of the PHS multi-year transition to an Epic-based electronic health record. Brigham and Women’s Hospital, Brigham and Women’s Faulkner Hospital, and Dana-Farber Cancer Institute converted to Epic on May 30th and the new Epic-related functionality is currently available for your patients at these institutions. Patient care information is available in Partners Physician Gateway at www.physiciangateway.org.
The transition from the Longitudinal Medical Record (LMR) to Epic for other PHS institutions is scheduled to be completed by the end of 2017. Until then, data from both Epic and the LMR will be available in the new Partners Physician Gateway.
You can continue to access:
- Visit and consult notes ▪ Radiology reports ▪ Discharge summaries
- Test results ▪ Medications ▪ Allergies
- Visit history ▪ Providers of care
- Inpatient and outpatient consult information
New features enable you to:
- Submit referrals for patients who have previously been seen at Partners HealthCare entities
- Manage your own users with the “Manage My Clinic” functionality
- Share a patient list
Additional features will be added in the coming months. Our physician liaisons can provide more information on Physician Gateway and help you to get the full benefit of its new features. Please email BWHPhysicianLiaison@partners.org to set up a time for a Brigham and Women’s physician liaison to contact your office.

MDCONNECT is a joint program of Brigham and Women's and Massachusetts General Hospital – founding tertiary care hospitals of Partners Healthcare System – and is a community physician's direct link to the transfer and transport services of these hospitals. With one call, MDCONNECT will coordinate timely communication between referring and receiving physicians, locate inpatient beds, and arrange transport. Ground-based, mobile ICUs, staffed with specially trained nurses and paramedics, are also available through this service. To transfer a patient to Brigham and Women's Hospital 24 hours-a-day, call toll-free 1-877-637-3337, press the number "2" for Brigham and Women's, and an MDCONNECT operator will facilitate all aspects of the transfer.

The Care Coordination Department provides discharge planning, utilization management, and psychosocial care services throughout a patient's stay. The process includes continuous communication between the patient and family, Brigham and Women's Hospital physician, the referring physician, and the insurer. Care Coordination staff assist all patients through their stay at our hospital, however, physicians recognizing a particular need for service prior to a patient's admission may make a referral directly to the department at the above numbers, Monday through Friday, 8:30am-5pm. During evenings and weekends, call the page operator at (617) 732-6600 and ask for the care coordinator on call.
