- Helping Incident
- Impression of CPE and Educational Goals
- Religious Life Development
- Documentation for immigration if you are an international student
- Students with previous CPE should also include self and supervisory final evaluations from previous units and your personal and professional goals
- $75.00 check to the Brigham Chaplaincy Fund (If you are applying for the Summer Unit, make the check payable to "MGH CPE Fund")
- 3 reference letters (these letters can either come in sealed envelopes with the application or can be mailed from the individuals providing your references). References should be one academic, one denominational, and one personal
