Application Guidelines

Please provide the following items with your application (and please send via e-mail):

1. Your name, office address, department name, office phone, office fax, and office e-mail address
2. A copy of your current CV
3. A brief essay (maximum two pages) describing:
   a) how you will use the award to promote ethics at BWH, and
   b) how the award will enhance your ability to contribute to ethics within your professional discipline and within the hospital community as a whole
   NOTE: We strongly encourage applicants to discuss concrete ways in which they anticipate bringing ethics to their work at BWH.
4. A letter of recommendation from your supervisor
5. A letter of recommendation from a staff member outside of your department or discipline. For example, if you are a nurse, you should elicit a recommendation from a staff member who is not a nurse

Current BWH Ethics Committee members are not eligible for this award.

Please return your completed application to Pamela Galowitz via e-mail (pgalowitz@partners.org). Letters of recommendation should be e-mailed directly from the person writing the recommendation.

The application deadline is  March 6, 2009. Recipients will be announced by the end of March..
