Referral Information
Patients should be encouraged to alert BWH office staff as to the primary care physician/dentist's name, as well as the referring physician/dentist, so that appropriate information can be provided to them in a timely fashion. Referral numbers, when needed, must be obtained prior to the appointment.
BWH is committed to developing a seamless transition between primary care and tertiary care referral. BWH physicians/dentists prefer to review all pertinent referral information, procedures and tests before ordering additional studies so as to avoid duplicate or unnecessary tests. Radiologic pictures rather than reports are preferred. Please fax, mail or deliver with the patient all primary lab tests, radiology films, angiographic data and a summary of medical problems and medications.
