As part of our ongoing service to referring physicians, we provide the following patient transfer referral options.
HVC Inpatient Navigator
Our trained HVC Inpatient Navigators will help facilitate your inpatient transfers as expeditiously as possible. Combining years of clinical nursing knowledge with access to all pertinent clinical and admitting systems, our navigators are trained to handle all Cardiology, Cardiac Surgery, and Vascular Surgery inpatient transfers. The HVC Inpatient Navigators manage the inpatient and procedural area patient flow and are experts in triaging patients, solutions, and referrals.
The HVC Inpatient Navigation Center (617-543-4170) is staffed Monday through Friday, 7 am – 7 pm.
After-hours transfers are handled by Mass General Brigham MD Connect (1-877-637-3337). MD Connect is a joint program between Brigham and Women’s Hospital and Massachusetts General Hospital, serving as a community physician's direct link to the transfer and transport services of these hospitals. With one call, MDCONNECT will coordinate timely communication between referring and receiving physicians, locate inpatient beds, and arrange transport. Ground-based, mobile ICUs, staffed with specially trained nurses and paramedics, are also available through this service.
