The Care Coordination Department provides discharge planning, utilization management, and psychosocial care services throughout a patient's stay. The process includes continuous communication between the patient and family, Brigham and Women's Hospital physician, the referring physician, and the insurer. Care Coordination staff assist all patients through their stay at our hospital, however, physicians recognizing a particular need for service prior to a patient's admission may make a referral directly to the department at the above numbers, Monday through Friday, 8:30am-5pm. During evenings and weekends, call the page operator at (617) 732-6660 and ask for the care coordinator on call.
