Diabetes Management Program

The Diabetes Management Program at Brigham and Women’s Hospital provides an individualized, patient-centered approach to diabetes management. Our goal is to help our patients achieve their diabetes goals and to empower patients and their families to become knowledgeable, active participants in their health care.

We provide a full range of diabetes services including inpatient consultative care, outpatient medical management and comprehensive diabetes self-management education. Non-pregnant adults are seen in several locations. Pregnant patients are seen in conjunction with obstetricians in the Diabetes in Pregnancy Clinic, located on the main campus of Brigham and Women's Hospital, and at the Fish Center for Women's Health.

Our clinical staff maintains close communication with our patients’ primary care doctors to make sure that care is well coordinated. We also maintain close working relationships with collaborating dietitians, clinical pharmacists and other specialists at Brigham and Women's Hospital who provide care for patients with diabetes.
