Brigham Health Family Care Associates
At Brigham Health Family Care Associates, we welcome patients of all ages and provide team based, patient and family centered care. We have built our clinic team with our patients' needs in mind and work together to ensure that all people feel equally welcomed and cared for in our space.
We have a bilingual staff (English and Spanish) and have interpreter services available for all other languages to better serve our patients. Our expanded multidisciplinary team includes a patient navigator, social worker, population health manager, care coordinator, community health workers, nurses and resource specialists. Together, we have helped our patients and families organize transportation, apply for food stamps, obtain health insurance, access local food banks and meal delivery services, connect with adult day programming, advocate for their children to receive special services in school, treat and refer for behavioral health needs and more.
We have particular expertise in taking care of young adults growing up with childhood illnesses and patients with intellectual disabilities and mobility restrictions. We expanded and relocated our clinic to a brand-new facility in Brookline in February 2020 to better serve all of our patients' needs.

With a focus on preventative medicine and chronic disease management, we provide inclusive and culturally responsive care to honor humanity with compassion.

Racial justice involves the creation and proactive reinforcement of policies, practices, attitudes, and actions that produce equitable power, access, opportunities, treatment, and outcomes for all people.
Our clinic team is opposed to all forms of institutional and structural racism and are dedicated to addressing the racial and social injustices that exist in our healthcare system. Currently, our residents are leading a project to incorporate discussions of racial and social justice into well child visits across all Brigham and Women's pediatric practices. We have committed to providing anti-racist educational resources (in both Spanish and English) for our pediatric patients and families, including books to take home with specifically racial justice titles and content.
Each year, we learn more about our patient community by participating in local health fairs, vaccination drives, and health education classes at community centers and most importantly listening to our patients. As we have recently relocated our clinic to Brookline in 2020, we have begun connecting with local community organizations including an anti-racism group to start to work with them on our shared mission of health equity and racial justice. As we learn more about our new neighborhood, we look forward to building more partnerships with community organizations to best serve our patients.
