The Pulmonary Vascular Disease Program provides individualized evaluation and treatment for patients with unexplained shortness of breath, pulmonary arterial hypertension, pulmonary hypertension associated with heart disease, COPD, interstitial lung disease, chronic thromboembolic disease, liver disease, HIV disease, and other conditions.

Treatment recommendations and follow-up care are developed through close collaboration with referring clinicians.

A Checklist for Your Visit

We would like to streamline your evaluation as much as possible. Here is a Medical Test Check List that expedites our evaluation of the cause of your shortness of breath at the time of your clinic appointment. If you have had any of these tests at an outside hospital, please bring all imaging (echocardiograms, CT scans, nuclear scans) on a disk to your appointment so we can review the pictures. All other tests (pulmonary function, electrocardiograms, and/or stress tests) can be faxed to us at least two days prior to your appointment.

Medical Test Checklist

  • Pulmonary Function Test (PFT) (required prior to first visit)*
  • Echocardiogram (required prior to first visit)*
  • Recent Physician Office Note from your referring physician
  • ECG
  • Cardiac Stress Test
  • Ventilation/Perfusion Scan of the Lung
  • Chest CT Scan
  • Sleep Study
  • Right Heart Catheterization
  • Left Heart Catheterization
  • Six-minute Walk

  • Clinic Referral from Insurance- if required
  • Current Medication List
