If you or someone you know is currently hospitalized and wishes to transfer care to our Neuro-hospitalist service at Brigham and Women's Hospital, please ask the attending physician to call our transfer center (MDConnect) at 877-MD-REFER (877-637-3337). Registration at Brigham and Women's Hospital (i.e., obtaining a medical record number and "blue card") can be done by calling the Admitting Department at 617-732-7450. What to Expect in the Hospital

If you are a neurology patient at Brigham and Women's Hospital, you will be meeting many members of our team including an attending neurologist (a Neuro-hospitalist), neurology residents, a physician assistant, nurses, therapists, social workers, and care coordinators. This multidisciplinary team works together to coordinate diagnosis and treatment in the hospital, and communication with outpatient providers and rehabilitation facilities that will continue the patient’s care following hospitalization.

The doctor supervising your neurologic care is a Neuro-hospitalist. A Neuro-hospitalist is a specialist who is an expert in diagnosing and treating diseases of the nervous system in the inpatient setting. We are committed to providing compassionate and cutting-edge care, and ensuring a smooth transition from the hospital to the outpatient setting.

When you are discharged from our inpatient service, our team will provide a summary of your hospital stay and recommendations for your ongoing neurological care to your primary care physician (PCP) and other caretakers. We can arrange follow-up in the outpatient neurology clinic at Brigham and Women’s Hospital, or communicate with neurologists closer to your home if follow up is preferred there. We encourage you to enroll in Patient Gateway in order to facilitate communication after your discharge. This system allows direct correspondence with Brigham and Women’s Hospital providers.
