When referring patients for lab work, or if you are a patient needing lab work, a completed lab requisition must accompany each patient to phlebotomy. Any type of specimen submitted to the laboratory that was obtained in the physician's office must also have orders in the laboratory/hospital information system, and the patient from whom the specimen was obtained must be registered in the system. The patient must have an up-to-date card when coming to the phlebotomy labs. Please note: hours subject to change, please call to verify.
