Notary Public

A Notary Public is available through the Patient and Family Relations department to authorize important documents, including:

The Notary Public will visit the patient in his or her room while the patient is staying at the hospital. A federal photo ID, such as a driver’s license, photo ID or passport is required. The documents must be signed in the presence of the Notary Public.

Please note, on Saturdays the Notary Public is not available to notarize documents on patient floors, but is available at the Bretholtz Center.
