Anticoagulation Management Service (AMS)

Brigham and Women’s Anticoagulation Management Service (AMS) is a pharmacist-run clinic that provides full anticoagulation management services for all oral anticoagulants including warfarin (Coumadin, Jantoven), dabigatran (Pradaxa), rivaroxaban (Xarelto), apixaban (Eliquis), edoxaban (Savaysa) and betrixaban (Bevyxxa). Brigham and Women’s AMS has been recognized as a Center of Excellence by the Anticoagulation Forum.

Patients are referred to our service by a Brigham and Women’s physician, usually the patient’s primary care provider or a specialist such as a cardiologist or hematologist for management of anticoagulation therapy. The AMS is a service dedicated to caring for patients on anticoagulation which is a class of medications also referred to as “blood thinners”. Our service is made up of pharmacists, pharmacy interns, and administrative personnel. Pharmacists will prescribe anticoagulation and required laboratory tests and work closely with the referring doctor under a Collaborative Practice Agreement.

Anticoagulants can be a challenging class of medications to take because they require you to be very diligent in taking them as prescribed to minimize the risk of developing a blood clot of bleeding. If you are taking too little or miss doses of your anticoagulant, you may be at increased risk of developing a blood clot. If you are taking too much or take extra doses of your anticoagulant, you may be at increased risk of bleeding. While being managed by our clinic, it is very important that you communicate often with the AMS so that we can decrease your risk of having an adverse event.

  • Selection of most appropriate oral anticoagulant therapy based on patient specific variables (including direct-oral anticoagulants and warfarin)
  • Thorough education on anticoagulation based on national patient safety goals
  • Assistance with getting your medication filled at the pharmacy
  • Ordering and following labs based on patient specific factors
  • Management of anticoagulants around procedures or surgeries
  • Assessment and tools to improve adherence
  • 24/7 emergency pager for triaging bleeding and clotting concerns

After hours: A pharmacist is on-call 24 hours a day via pager # 17541 to assist with emergencies that cannot be handled during normal business hours
