Medication safety staff work closely with interdisciplinary members consisting of physicians, nurses, information systems experts and quality specialists who oversee medication safety projects in the continuous evaluation of medication use system issues and medication errors and near misses. The staff facilitate system changes to promote patient safety, equity and continuous quality improvement that reflect medication safety best practices. Medication safety staff also help to ensure compliance with accreditation and regulatory standards.
Medication safety staff in coordination with interdisciplinary members of hospital leadership are responsible for the review, collation and analysis of all inpatient and ambulatory medication related events. The medication safety staff provides continual follow-up and monitoring of medication error trends and smart pump data. Medication safety staff are also responsible for reporting significant medication safety findings and trends to pertaining committees including the Medication Report Review Committee, the Drug Safety Subcommittee of the Pharmacy and Therapeutics Committee, and the Executive Patient Quality and Safety Committee as needed.
Medication safety staff are integral in identifying and implementing priorities for quality improvement and information system changes. These staff members assist with assuring appropriate follow-up and system application of related processes associated with medication safety improvements. Additionally, they provide medication safety educational opportunities for PGY1 and PGY2 pharmacy residents through a medication safety elective rotation and medication safety related committee involvement.
