Research and Innovation: Redesign of Primary Care and Implementation Science

Rising health care costs highlight the need for primary care as the foundation of comprehensive population health management. Together, the care team brings a broader array of skills and expertise than any individual provider can offer. As with all teams, there must be considerable attention to defining and coordinating the activities and responsibilities of each team member individually, and as a group. It is the mission of the Center of Excellence to utilize implementation science to best understand the interventions with the greatest impact, and to implement the optimal team approaches for our patients.
