The Care Team and the Patient-centered Medical Home

Our practices have been recognized as Level 3 NCQA Patient Centered Medical Homes. Medical Home Recognition is based upon a practice proving, through systems and documentation, that they have a model of care that is based around quality and safety. Quality is achieved through enhancing communication and monitoring patient outcomes. The guiding principles of a Medical Home result in a more personalized, coordinated, effective and efficient care of patients.

The model advances the use of systematic processes and IT to enhance quality care of patients and promotes the principles of a Medical Home: Personal physician, physician-directed medical practice, whole person orientation, coordinated and integrated care, quality and safety and enhanced access. NCQA Recognized medical homes clearly demonstrate that systems and processes are in place to meet nationally recognized standards for delivering high-quality care.

We are committed to providing you with the highest quality care possible at all of our primary care locations. You will find that we operate utilizing a team-based care model. This means that all members of the staff at your primary care practice work together to manage and coordinate your health care, both during and between your visits with us.

Care Team members may include your Primary Care Provider, Physician Assistant, Nurse Practitioner, Medical Assistant, Nurse, Practice Assistant, Social Worker, Health Coordinator, Nutritionist, or Pharmacist; the Care Team works together to address your unique health care needs.
