How To Prepare For Your First Appointment
If you are a new patient of Urology, please bring the following to your appointment:
- Copies of Imaging CDs/Films (MRI, CT, X-rays), Test Results, and Office Notes from any outside facilities
- Insurance Card and Photo ID: Please bring your most recent valid insurance card. We will need to make a photo copy of your insurance card and photo ID for our records even if your insurance information is on file.
- Referral: If you are a member of an HMO or managed care plan, please call your primary care provider's office prior to your visit to notify them that you have an appointment with a BWH Urologist. Our office must receive your referral prior to your appointment. Our billing coordinator can be reached at (617) 732-4952 and our referral fax number is (617) 525-9894.
- Co-payment: Your co-payment will be collected at the time of your visit, located on your insurance card. We accept checks and all major credit cards. Checks can be made out to BWPO (Brigham and Women's Physicians Organization). Please note that as of May 30, 2015, we will no longer accept cash payments in clinic.
