Perinatal Case Manager Program

For more than 20 years, our Perinatal Case Manager Program has provided a direct response to the high infant mortality and low birth weight rates in certain Boston neighborhoods. The Perinatal Case Manager Program seeks to prevent infant deaths and poor birth outcomes by addressing the social and medical needs of pregnant women. We assist pregnant women with everything from coordinating their health care to learning about healthy behaviors and accessing vital social service programs.

CCHHE provides technical assistance and training for case managers at six of our hospital’s licensed and affiliated health centers: Brookside Community Health Center, Martha Eliot Health Center, Mattapan Community Health Center, Southern Jamaica Plain Health Center, South End Community Health Center, and Whittier Street Health Center. By working together with hospital providers, case managers ensure that culturally responsive care continues for pregnant women throughout their perinatal period.

The Perinatal Case Managers provide a variety of services to pregnant women, including:

  • Assessment of patients’ needs
  • Supportive referrals to appropriate social services
  • Coordination of patient care with other health center and hospital providers
  • Assistance in overcoming barriers to accessing health care and social services
  • Education about the need for preventive care and healthy behaviors
  • Financial assistance

Our Perinatal Transportation Assistance Program (P-TAP) addresses the impact the lack of transportation can have on the ability of pregnant patients to access prenatal care. P-TAP assists pregnant and postpartum women in getting to and from their perinatal appointments by providing access to MBTA Charlie Cards and/or taxi vouchers. In addition to the six health centers served by the Perinatal Case Manager Program, patients from four additional health centers affiliated with Brigham and Women’s Hospital – Codman Square Health Center, Dorchester House Multi-Service Center, Neponset Health Center, and Upham's Corner Health Centers – have access to this resource.

The Perinatal Case Manager Program offers an Infant Car Seat program to ensure that any woman who delivers at Brigham and Women’s Hospital and who is unable to afford an infant car seat will receive one. Patients are asked to pay a nominal fee for the seat; however, in cases where the patients are unable to pay, this fee can be waived. Eligible patients must attend a one-hour group training session on car seat safety prior to receiving a car seat.
