The RN Care Coordinator manages a caseload of patients on a defined service and is responsible for ensuring care that supports desired clinical and financial outcomes.
Coordinates and insures implementation of the plan of care.
Coordinates and executes the dicharge planning process for patients, ensuring each patient has a discharge plan.
Collaborates with appropriate individuals, departments, and payers to insure appropriateness of admission, continued days of stay, and reimbursement.
