Make an Appointment in the Advanced Heart Disease Center
Referrals may be initiated by patients, primary care physicians, or cardiologists either for outpatient evaluation or inpatient hospital transfer. Subsequent care will include assessment by advanced heart disease faculty and heart failure nurse specialists, with continued involvement as requested by the patient and referring physician.
The Advanced Heart Disease Center evaluates over 400 new patients each year. Even for the most severe cases of heart failure, the systematic approach to therapy has been proven to improve cardiac function, increase exercise performance, enhance quality of life, and decrease re-hospitalization for many patients.
