Patients are evaluated for sleep disorders in our Sleep Clinic at the Sleep HealthCenter affiliated with Brigham and Women's Hospital in Brighton (Boston). Clinicians trained in Sleep Medicine obtain a history of the sleep complaint (ideally this includes a history from the patient's bed partner); medical, psychiatric, and family history; medication use history; psychosocial situation assessment; and a physical examination. In some cases, a definitive diagnosis can be made and treatment can be prescribed, whereas in other cases the diagnosis is presumptive or even uncertain. If the diagnosis is presumptive, it must be confirmed or excluded by diagnostic studies. These diagnostic studies may include over-night polysomnography (a study designed to diagnose sleep disorders). If the diagnosis is uncertain, a differential diagnosis is formulated and a plan is constructed to determine a definitive diagnosis. Common chief complaints are insomnia (difficulty initiating or maintaining sleep), excessive sleepiness, snoring, nocturnal movements, and altered behaviors during sleep. Patients diagnosed with sleep disorders are also managed in our Sleep Clinic.
