PACT's team of health promoters, trained in education, counseling, and case management, work with participants to improve their access to quality health and social services and, in turn, improve their health and well-being.
One-on-One Support for Patients with HIV
Our health promoters take a holistic approach to the care of their patients – getting to know them within their homes and communities, accompanying them as they negotiate the health and social service system, translating treatment recommendations, and helping participants embrace healthier lifestyles and positive outlooks.
Our unique relationship with our clients helps them identify barriers that impede their health and motivate them to adopt behaviors conducive to maintaining health and comfort.
A Comprehensive Approach
Our intensive and collaborative approach to community-based health promotion allows us to accomplish our mission: to provide a comprehensive approach to HIV education and prevention within the community and to ensure that all people living with HIV/AIDS have access to quality health care and social services.
