BWH Summer Intern On-Boarding Process
Please note: All summer interns who are working in research laboratories should follow the process outlined on the Research Trainee page.
All Summer Interns must attend an orientation, be cleared by BWH Occupational Health and Receive a BWH ID badge prior to beginning his/her summer internship
1) The BWH department, hosting the Summer Intern, will schedule the applicant to attend a Sponsored Staff Orientation (SSO) by sending an email to bwhoss@partners.org Summer Interns can attend either a new SSO or a Summer Intern Orientation (See Orientation Schedule for dates). Space is limited and registrations are taken on a first come, first served basis. Email should include the following:
a. Summer Intern in subject line
b. Desired SSO date
c. Applicant name
d. BWH department
e. BWH employee contact name and extension
f. Funding source, or institution giving academic credit, if applicable
g. Visa information, if applicable. (Please be aware that the Office for Sponsored Staff and Volunteer Services (OSSVS) does not work with or facilitate any part of the visa acquisition process. It is the responsibility of the department to ensure that the person they are interested in bringing into the hospital as a Sponsored Staff has the appropriate visa to work in the United States and within the hospital. 2) The BWH department will forward the following documents to the Summer Intern. These documents must be completed, by the applicant and the department, prior to the applicant's attendance at SSO. The applicant will bring the completed documentation to SSO.
a. Sponsored Staff Registration Form
b. Confidentiality Agreement
c. CORI Request Form
d. Research Trainee Letter
e. Government issued picture ID (Acceptable forms of identification can be found at the end of an I-9 form, which you can view on the following website: http://www.uscis.gov/files/form/I-9.pdf )
f. Visa information, if applicable
g. Immunization records (Sponosred Staff Medical Screening Form may be completed by applicant's physicician: please view both pages of the form for further instructions.)
h. Recent TB-test, plant and result (received within the last 90 days)
3) The applicant attends the entire SSO with above mentioned documentation. Applicants who are more than 20 minutes late will not be allowed to attend the Orientation.
4) The applicant receives clearance from BWH Occupational Health (OSSVS will facilitate this process).
5) The applicant receives a BWH Sponsored Staff ID badge (OSSVS will facilitate this process) and is eligible to begin at BWH.
Note: In the event that the Student Intern you are on-boarding is a minor (age 16-17) s/he will also need to bring the following additional forms to SSO:
a. A completed and signed Massachusetts Work Permit. The form can be obtained online at the following web-site: http://www.mass.gov/Elwd/docs/dos/youth_employment/youth_application.pdf
b. A completed and signed Medical Screening Minor Consent Form
c. A completed and signed Laboratory Minor Consent Form if the minor will be in a laboratory setting at BWH.
