Insurance Coverage
Unfortunately, third party insurance plans do not cover our program fees. Our program is a self-pay program and we are unable to bill insurance companies for our services.
If you believe that your insurance company will cover the cost of the Program for Weight Management, we will be happy to provide you with any receipts and/or medical documentation to assist you with your efforts. It will be your full responsibility to submit and collect any insurance reimbursement.
All payment is due prior to delivery of services. We accept cash, Master Card, Visa, and American Express. Sorry, but we are not able to accept personal checks.
All fees are effective as of April 30th, 2002
