Center for Older Adult Health Services

The Center for Older Adult Health is geared to meet the needs of patients 65 and older who require consultative care. The clinical team works with patients to optimize overall health and to achieve the goals important to that individual – from helping patients maintain good health to treating acute and chronic conditions. This includes determining the best way to manage multiple conditions, which sometimes affect older patients.

We offer:

  • Consultative geriatric care
  • Medication assessment
  • Functional assessment
  • Memory and cognitive skills evaluation
  • Dementia care for patients and caregivers
  • Advance care planning
  • Referrals as needed.

We are skilled in helping patients with/to:

  • Falls and mobility issues
  • Memory problems
  • Alzheimer's disease
  • Depression
  • Weight loss
  • Failure to thrive and nutritional issues
  • Urinary incontinence
  • Achieve the best possible quality of life.

Establishing care with our providers:

Patients need to have an established general primary care physician at one of these Brigham and Women’s Hospital clinic sites before scheduling with a geriatrician:

New primary care physicians are available at these practice locations. Please contact the preferred location to schedule with a general primary care physician. Once an appointment is confirmed with the primary care physician, a new appointment can be scheduled with the coordinating geriatrician.
