South Huntington Primary Care: A Patient Centered Medical Home

The Patient-Centered Medical Home (PCMH) is a care delivery model to improve health care. Medical homes provide coordinated, patient-centered care to the patients they serve. The model strives to provide comprehensive, accessible, and quality care. The goal is to help patients receive the necessary care when they need it, where they need it, in a way that the patient understands. It helps patients build relationships with the members of their clinical team. To learn more about how a PCMH works, watch this short video by EmmiSolutions. You can also read more about it at the Patient Centered Primary Care Collaborative website.

The National Committee for Quality Assurance (NCQA) is the most well know PCMH recognition program and there are more than 12,000 practices that are recognized. For more information you can visit their website. As a level 3 (highest level) PCMH, our clinic at South Huntington has made a commitment to providing great care.
