SAGE Program Honored With 2016 Success Story Award

The Speaking About Goals and Expectations (SAGE) program at BWH has been named one of 15 recipients of the 2016 Success Story Award by Press Ganey. The award, presented at the annual National Client Conference, honors Press Ganey clients that have demonstrated leadership and innovation to improve patient experience, clinical quality or caregiver engagement.

The Brigham and Women’s Physicians Organization supports and implements the SAGE program, with an interdisciplinary leadership team. The SAGE program is a primary palliative care intervention aimed at improving the lives of BWH patients hospitalized with serious illness by facilitating conversations about values and priorities, while delivering high-quality, coordinated care that is aligned with patients’ goals and wishes. SAGE provides intensive serious illness communication skills training for inpatient hospitalist providers, along with the support of a clinical social worker, Catherine Arnold, LICSW, who collaborates with inpatient teams to ensure that information is communicated to outpatient providers and that patients and families have a contact for concerns and support after discharge.
