Student Intern On-boarding Process

Orientation space is limited; please register student interns two weeks in advance of desired orientation date. Incomplete orientation requests and requests received less than two business days prior to the requested orientation date will not be processed. Per Occupational Health, OHS appointments will not be scheduled nor clearance given until OSSVS has received complete immunization records and proof of TB tests.

  1. 1. Schedule your student intern to attend a NSSO by sending the following information to bwhoss@partners.org:
    1. Desired NSSO date
    2. POI form
    3. Signed letter on letterhead from academic institution verifying internship is a program requirement (if applicable) OR Intern Curriculum
    4. Quick Registration Form
    5. Copy of government issued photo ID
    6. Visa information and documents if applicable (I-20, DS-2019, I-797 etc)*
    7. Immunization records including TB plant and result (received within the last 90 days) Health Screening Requirments form may be completed by the applicant's physician; please view both pages of the form for further instruction.

      *Please remember to verify that the person you are hiring has the appropriate visa and work authorization prior to beginning the on-boarding process. Complete immunization records and proof of two Tb tests, one within a year and one no older than 90 days (2 TB tests are REQUIRED).
  • 2. The new student intern will receive an email from bwhoss@partners.org with a link to the HealthStream training, their username, password and instructions for completing HealthStream.
  • 3. The new student intern will bring these following documents to orientation:
    1. US government issued picture ID or a valid ORIGINAL foreign passport
    2. Copy of HealthStream transcript
    3. Swipe card access form
  • 4. After attending the NSSO with above mentioned documentation, the intern will receive clearance from BWH Occupational Health and will obtain a BWH ID badge (OSSVS will facilitate both processes) and is eligible to begin at BWH.

    Keep in Mind: In the event that the Student Intern you are on-boarding is a minor (age 16-17) s/he will also need the following:

    2. Medical Screening Minor Consent Form
    3. Consent to Evaluate and Treat Form
    4. CORI Parental Consent Form
