You must submit an Application for Renewal of Privileges if your role will continue exactly as your original credentialing. The Application for Renewal of Privileges form must be signed by the school of nursing and your immediate supervisor (if you are a BWH RN). a. If you teach at BWH for 2 different schools, you will need to submit a separate packet for each school.
POI Form: We are required to have a completed Person of Interest (POI) form for all Clinical Instructors. If your credentials have expired (greater than 1 year) or we do not have a POI form in your file, we will include this in your reminder. Fill out and sign the top portion of this document. The CNE completes the remainder of the form. Incomplete POI Forms will be returned to applicant.
BLS Provider Card: Please include a copy of your current BLS card.
HealthStream: The BWH Compliance Office requires that all staff and POIs complete the annual on-line HealthStream Compliance competencies (HIPAA, fire safety, etc.), this includes the school of nursing clinical instructors (and students). Failure to complete these prior to the due date will result in loss of access and deactivation of your account. Please remember to check your HealthStream assignments before, during and after the semester so you do not lose access if you intend to continue teaching at BWH in the future.
Going forward, your credential renewal will be due on your birthday annually:
Your renewal is due at least 6 weeks in advance of the expiration date.
You will receive a reminder email from BWHNursingCredentialing@partners.org at 8 weeks and 4 weeks beforehand.
We will include the school on subsequent reminders sent to you until you submit your paperwork or notify us that you are no longer teaching for that school.
You will receive an email informing you that we have archived your file if you do not renew within 4 weeks past your expiration date.
You must submit a full “New” Application for Privileges:
If you have contracted to teach for an additional school.
If your credential privileges have expired more than one year.
