For Brookside Community Health Center Patients and Families
We are honored and privileged that you are considering Brookside Community Health Center. We pride ourselves on providing the highest quality, patient-centered care to those in Jamaica Plain and the surrounding Boston communities. To become a new patient and choose a provider, please call 617-522-4700 for more information and to schedule your first appointment.
Our staff is well-equipped and ready to assist you in registering as a new patient, updating your information in the system, and providing you with key information as it relates to HIPAA, Patient's Rights and Responsibilities, and Consent to Treat.
If registering for the first time, please arrive 30 minutes prior to your scheduled first appointment and please bring the following items with you:
Photo Identification (if registering a child, photo ID of parent/legal guardian)
If insured, medical insurance card.
We accept many health insurance and managed care plans, including MassHealth (Medicare), Neighborhood Health Plan, Medicaid, and all Medigap (Medicare Supplement) plans. We also accept Delta Dental, DentaQuest, BCBS of MA, and Cigna Dental for our on-site dental services. For a list of plans that we accept, please click here. This list is updated periodically and subject to change, so please check with your health plan to verify coverage. You may also contact Brookside's Finance Department and speak with our Financial Counselors who are also Affordable Care Act (ACA) Certified Application Counselors. If we do not accept your insurance, but you would like to become a Brookside patient, please contact our Financial Counselors for assistance with options available.
Have no medical health insurance? No problem. Our Financial Counselors can also assist you with applying for medical and dental coverage through MassHealth and the Massachusetts HealthConnector.
