Family Medical Department

The Family Medical Department provides primary care for patients of all ages. By providing patient-centered care, the care team works to develop lifelong partnerships between our providers, our staff and our patients. Our care teams consist of primary care providers, RNs, LPNs, Medical Assistants, and Practice Assistants to provide comprehensive care. Population Health Managers, Community resource specialists, a pulmonologist, and a cardiologist collaborate with our care teams to support our patients. Patients receive care during face to face visits with providers, RNs, and LPNs; Home care advice and care coordination are provided through telephone triage and support; and on the internet portal called Patient Gateway. We take pride in providing high quality medical care for the residents of Boston in a friendly, patient-centered environment.

• Fitness in the City: Provides free nutrition counseling and physical activity resources for eligible pediatric patients ages 5-18 years.
